SINBAD Score for Diabetic Foot Ulcer

Simple 6-domain diabetic-foot ulcer classification (Ince 2008) — recommended by IWGDF for international comparison. Each domain 0/1; total 0–6.

Score interpretation

SINBAD 0–2 — Lower Severity 0–2

Below the ≥ 3 threshold at which healing times step up (Ince 2008).

→ Diabetic foot protection / multidisciplinary foot service per NICE NG19; off-loading; local wound care; glycaemic optimisation; reassess the score as the ulcer changes.

SINBAD ≥ 3 — Higher Severity 3–6

Score ≥ 3 of 6: longer time to healing and more amputations (Ince 2008).

→ Multidisciplinary diabetic foot service per NICE NG19 (urgently if ischaemia or infection). Investigate ischaemia (ABPI, duplex), screen for osteomyelitis (probe-to-bone + X-ray); revascularisation pathway and IV antibiotics as needed; counsel about amputation risk.

Interpretation bands for the SINBAD. Apply clinical judgement and local guidance.
