Predicts 30-day potentially avoidable hospital readmissions using seven variables available at discharge.
Score interpretation
HOSPITAL 0–4: Low risk (~5% 30-day readmission)
→ Standard discharge planning; GP follow-up within 7 days; discharge summary
HOSPITAL 5–6: Intermediate risk (~12% 30-day readmission)
→ Structured discharge; post-discharge phone call; medication reconciliation; early outpatient appointment
HOSPITAL ≥7: High risk (~20%+ 30-day readmission)
→ Intensive discharge planning; care coordinator; home visit; transition care programme; consider brief observation stay or step-down facility
