ISAR — Identification of Seniors At Risk

6-item ED screening tool for older adults (McCusker 1999) predicting 30-day adverse outcomes (functional decline, readmission, institutionalisation, mortality). Score ≥2 triggers comprehensive geriatric assessment.

Score interpretation

Low risk 0–1

→ Standard ED disposition. No additional geriatric input required.

High risk — refer for CGA 2–6

→ Refer to ED-frailty / Older Person's Assessment & Liaison (OPAL) team. Avoid prolonged waits in trolley areas. Review medications (STOPP/START), capacity, falls, social support and discharge planning before leaving ED. Front-door frailty pathways shown to reduce admission and improve outcomes (BGS / RCEM 2024).
