Low adherence risk
→ Self-administered therapy appropriate; regular clinic review
Moderate adherence risk
→ Enhanced support: social worker involvement, video observed therapy (VOT), pill counting
High adherence risk
→ Directly observed therapy (DOTS) required; community TB nurse; address social barriers; consider inpatient initiation
