Neonatal Jaundice — Bhutani Nomogram Risk
Assesses risk zone of neonatal bilirubin level based on age in hours. Guides phototherapy and exchange transfusion decisions per NICE/AAP.
Score interpretation
Low zone / Low-intermediate zone
→ Routine monitoring; repeat bilirubin in 12-24h; encourage feeding; reassess at discharge
High-intermediate or near phototherapy threshold
→ Plot on treatment threshold chart; start phototherapy if at/above threshold; check blood group, DAT, FBC
High zone or risk factors present
→ Urgent phototherapy; check G6PD, reticulocytes; consider exchange transfusion if rising rapidly; neonatology review
