Score interpretation

No Significant Withdrawal 0–4

Score 0–4

→ Monitor; not in sufficient withdrawal for buprenorphine induction

Mild Withdrawal 5–12

Score 5–12: Mild opioid withdrawal

→ Symptomatic management; buprenorphine induction can begin at score ≥8–12

Moderate Withdrawal 13–24

Score 13–24: Moderate opioid withdrawal

→ Buprenorphine induction indicated; continue dose titration

Moderately Severe / Severe 25–48

Score ≥25: Moderately severe to severe withdrawal

→ Prompt buprenorphine/methadone treatment; supportive IV fluids if needed
