CHARGE-AF Score for Incident Atrial Fibrillation

Cohort for Heart and Aging Research in Genomic Epidemiology (CHARGE) AF score. Predicts 5-year risk of developing new-onset atrial fibrillation in patients in sinus rhythm.

Derived in adults aged 46–94 years.

The model was derived in White and African American adults only; other groups are not validated.

Score interpretation

Low 5-Year AF Risk (< 2.5 %) 0–2.499

Predicted 5-year risk of incident AF below 2.5 %.

→ Optimise cardiovascular risk factors; reassess if risk factors change.

Intermediate 5-Year AF Risk (2.5–5 %) 2.5–5

Predicted 5-year risk of incident AF 2.5–5 %.

→ Address modifiable risk factors (blood pressure, weight, alcohol, sleep apnoea); opportunistic pulse checks.

High 5-Year AF Risk (> 5 %) 5.001–100

Predicted 5-year risk of incident AF above 5 %.

→ Intensive risk-factor management; consider ECG screening for AF where the result would change management.

Interpretation bands for the CHARGE-AF Score. Apply clinical judgement and local guidance.
