Predicts major bleeding risk in atrial fibrillation patients on anticoagulation.

Used in: Atrial Fibrillation

Prior GI bleed, intracranial haemorrhage, or haemorrhagic stroke

Hb <13 g/dL (men) or <12 g/dL (women), OR Haematocrit <40% (men) or <36% (women)

Score interpretation

Low Bleeding Risk 0–2

ORBIT 0–2: 2.4 major bleeds per 100 patient-years.

→ Proceed with anticoagulation. Standard monitoring.

Medium Bleeding Risk 3

ORBIT 3: 4.7 major bleeds per 100 patient-years.

→ Anticoagulate with caution. Address modifiable risk factors. Close follow-up.

High Bleeding Risk 4–7

ORBIT 4–7: 8.1 major bleeds per 100 patient-years.

→ Carefully weigh bleeding risk vs stroke prevention benefit. Address modifiable risk factors. Consider haematology review.
