EUROMACS-RHF Score for Right Heart Failure after LVAD

Predicts right heart failure (RHF) after left ventricular assist device (LVAD) implantation. Identifies patients at high risk who may benefit from RVAD or early optimisation.

Score interpretation

Low Risk (<10% RHF)

→ EUROMACS-RHF Low Risk (0–2): Low risk of right heart failure post-LVAD. Proceed with standard LVAD protocol. Post-op RV assessment and optimisation.

Intermediate Risk

→ EUROMACS-RHF Intermediate Risk (3–4): Moderate RHF risk. Consider preoperative RV optimisation (milrinone, inhaled nitric oxide). Plan for prophylactic RVAD placement if clinical deterioration. LVAD centre MDT review.

High Risk (>30% RHF)

→ EUROMACS-RHF High Risk (≥5): High risk of RHF. Consider alternative therapies (cardiac transplant); prophylactic RVAD or BiVAD; inhaled nitric oxide perioperatively; intensive post-op RV management in cardiac ICU.
