EUROMACS-RHF Score for Right Heart Failure after LVAD
Predicts severe right heart failure (RHF) requiring short-term mechanical circulatory support after LVAD implantation.
Score interpretation
EUROMACS-RHF 0–2: Low risk of severe right heart failure post-LVAD. 1-year survival good.
→ Standard LVAD perioperative protocol.
EUROMACS-RHF 3–5: Intermediate risk. Right heart monitoring important post-LVAD.
→ Pre-emptive right heart optimisation. Close postoperative RV monitoring. RVAD availability.
EUROMACS-RHF ≥ 6: High risk of severe RHF requiring MCS. 1-year mortality substantially elevated.
→ Consider BiVAD or total artificial heart. Cardiac transplant assessment. Defer LVAD if possible.
