PCP-HF Risk Score (Pooled Cohort Equations to Prevent Heart Failure)

10-year risk prediction model for incident heart failure in patients without current HF. Uses clinical and laboratory variables to identify patients who may benefit from primary HF prevention strategies.

Low 10-Year HF Risk (< 5%) 0–1

Estimated 10-year HF risk < 5% -- primary prevention standard measures

→ Lifestyle modification: aerobic exercise >= 150 min/week moderate intensity; DASH diet (dietary approaches to stop hypertension); weight loss if BMI > 25; smoking cessation; alcohol moderation (< 14 units/week); BP target < 130/80 mmHg (AHA 2017 guidelines) or < 140/90 mmHg (NICE 2019); reassess every 5 years or when risk factors change; no specific HF prevention pharmacotherapy indicated at this risk level.

Intermediate 10-Year HF Risk (5-10%) 2

Estimated 10-year HF risk 5-10% -- intensified risk factor management

→ Intensify lifestyle interventions; ACEi/ARB for hypertensive patients (reduce incident HF -- HOPE trial: ramipril reduced HF); SGLT2 inhibitor for diabetic patients (empagliflozin, dapagliflozin -- reduced HF hospitalisation even without established HF); statin for dyslipidaemia (CORONA trial; WOSCOPS: pravastatin reduced incident HF); BMI target < 30 kg/m2; 3-monthly BP review; HbA1c target < 53 mmol/mol (7%); BNP or echo screening if clinical concern (ACC/AHA: consider screening if multiple risk factors).

High 10-Year HF Risk (> 10%) 3

Estimated 10-year HF risk > 10% -- aggressive primary prevention required

→ SGLT2 inhibitor (dapagliflozin/empagliflozin) regardless of diabetes if eGFR >= 45 and high CV risk (DAPA-HF, EMPEROR-Reduced, SOLOIST-WHF); ACEi or ARB for hypertension/high-risk profile; spironolactone for resistant hypertension (PATHWAY-2 trial); BNP-guided therapy: if BNP > 50 pg/mL -- start ACEi/ARB/beta-blocker/spironolactone to prevent HF development (STOP-HF trial); echocardiogram to screen for subclinical LV dysfunction; cardiology referral for primary prevention strategy; wearable devices for activity monitoring; 6-monthly review with cardiovascular risk reassessment.
