PRISMA-7 Frailty Screening Tool

7-item self/informant-report frailty screen for community-dwelling older adults (Raîche 2008). Validated in primary-care, ED triage and population screening. Score ≥3 indicates frailty and triggers comprehensive geriatric assessment.

Score interpretation

Not frail 0–2

→ Routine review. Encourage exercise, social engagement, nutrition. Re-screen at 12–24 months.

Frail — needs CGA 3–7

→ Comprehensive geriatric assessment (BGS Toolkit). Review polypharmacy (STOPP/START), nutrition (MUST), cognition (AMTS / 6-CIT), continence, mood, falls, advance care planning, social support. Onward physiotherapy / occupational therapy / community-frailty MDT referral.
