Alvarado Score for Acute Appendicitis
MANTRELS score for predicting acute appendicitis. Widely used in emergency settings to guide clinical decision-making and imaging requests.
Score interpretation
Score 0–4 — low probability of appendicitis. Unlikely diagnosis.
→ Appendicitis unlikely; observe; discharge with return precautions; consider alternative diagnoses
Score 5–6 — equivocal. Appendicitis possible.
→ CT scan abdomen/pelvis recommended; observe; surgical review if worsening
Score 7–8 — probable appendicitis
→ Urgent surgical review; CT scan if diagnosis uncertain; prepare for appendicectomy
Score 9–10 — clinical appendicitis
→ Surgical admission; appendicectomy; pre-operative antibiotics
