Tokyo Guidelines Cholangitis Severity (TG18)
Classifies severity of acute cholangitis using the 2018 Tokyo Guidelines (TG18) to guide timing of biliary drainage and antibiotic therapy.
Score interpretation
Mild (Grade I) Cholangitis
→ Responds to antibiotics; elective biliary drainage within 24-48h; ERCP or PTBD
Moderate (Grade II) Cholangitis
→ Early biliary drainage within 24h; IV antibiotics; monitor organ function
Severe (Grade III) Cholangitis
→ Organ dysfunction present; urgent ICU; emergency biliary drainage (ERCP or PTBD); broad-spectrum antibiotics
