International Staging System for Multiple Myeloma (ISS)

ISS staging system for multiple myeloma based on serum beta-2 microglobulin and albumin. Predicts overall survival. See also R-ISS for cytogenetic risk stratification.

Score interpretation

Stage I

→ ISS Stage I: β2M <3.5 mg/L AND Albumin ≥3.5 g/dL. Median OS ~62 months. Standard systemic therapy; discuss transplant eligibility.

Stage II or III — check albumin

→ β2M 3.5–5.4 mg/L: Stage II if albumin ≥3.5 g/dL (median OS ~44 months) or Stage II if not meeting Stage I/III criteria. Verify albumin.

Stage III

→ ISS Stage III: β2M ≥5.5 mg/L. Median OS ~29 months. High-risk disease; consider clinical trial, novel agents, early transplant if eligible.
