Erasmus GBS Outcome Score (EGOS)

Erasmus GBS Outcome Score (EGOS): predicts the probability of being unable to walk independently 6 months after Guillain–Barré syndrome, from age, preceding diarrhoea and the GBS disability score 2 weeks after admission. Derived in patients unable to walk independently.

Score interpretation

EGOS 1–3 — Lower Predicted Risk 1–3

Lower predicted risk of being unable to walk independently at 6 months (1 % at EGOS 1).

→ IVIG (adults: 0.4g/kg x 5 days) or plasmapheresis; physiotherapy; pain management; DVT prophylaxis; most patients walk at 6 months. Children: use a children's formulary.

EGOS 3.5–5 — Intermediate Predicted Risk 3.5–5

Intermediate predicted risk of being unable to walk independently at 6 months.

→ IVIG or plasma exchange; ICU monitoring; respiratory function monitoring (FVC, NIF); may need ventilation; rehab planning; bladder care; nutritional support

EGOS 5.5–7 — High Predicted Risk 5.5–7

High predicted risk of being unable to walk independently at 6 months (83 % at EGOS 7).

→ ICU admission; ventilatory support if FVC <20ml/kg or NIF <30cmH2O; autonomic monitoring (arrhythmia, BP swings); prolonged rehab; second IVIG if plateaued; tracheostomy if prolonged ventilation
