Erasmus GBS Outcome Score (EGOS)

Predicts probability of independent ambulation at 6 months in Guillain-Barre syndrome. Guides ICU admission and rehabilitation planning.

Score interpretation

Good prognosis — likely independent at 6 months

→ IVIG 0.4g/kg x 5 days or plasmapheresis; physiotherapy; pain management; DVT prophylaxis; most patients walk at 6 months

Intermediate prognosis

→ IVIG or plasma exchange; ICU monitoring; respiratory function monitoring (FVC, NIF); may need ventilation; rehab planning; bladder care; nutritional support

Poor prognosis — ventilation likely or prolonged disability

→ ICU admission; ventilatory support if FVC <20ml/kg or NIF <30cmH2O; autonomic monitoring (arrhythmia, BP swings); prolonged rehab; second IVIG if plateaued; tracheostomy if prolonged ventilation
