Estimated Fetal Weight (Hadlock)
Estimates fetal weight from ultrasound measurements using Hadlock 4-parameter formula.
Score interpretation
EFW < 500g: Periviable / extreme prematurity range.
→ Fetal medicine referral. Parental counselling regarding viability. Serial growth surveillance.
EFW 500–2499g: Low for gestational age or preterm. Assess against customised or population centile charts.
→ Plot on centile chart. If < 10th centile: refer for fetal wellbeing assessment (Doppler, BPP). Growth USS 2-weekly.
EFW 2500–3999g: Normal term range.
→ Correlate with gestational age and centile chart. Routine antenatal care if tracking normally.
EFW ≥4000g: Macrosomia. Increased risk of shoulder dystocia and operative delivery.
→ Diabetic screen if not done. Discuss birth plan. Consider induction if EFW >4500g or symptomatic. Shoulder dystocia simulation recommended for attending team.
Interpretation bands for the EFW (Hadlock). Apply clinical judgement and local guidance.
