Paediatric Sepsis Criteria (pSOFA)

Paediatric Sequential Organ Failure Assessment (pSOFA). Adapted from adult SOFA; assesses organ dysfunction across 6 systems for paediatric sepsis severity and mortality prediction.

Score interpretation

pSOFA < 2 0–1

pSOFA below 2.

→ Reassess if the clinical picture changes.

pSOFA ≥ 2 — Organ Dysfunction 2–24

pSOFA 2 or more indicates organ dysfunction (Sepsis-3 adapted for children, Matics 2017); with infection, consider sepsis. The Phoenix criteria (2024) are the newer paediatric sepsis definition.

→ Escalate per local paediatric sepsis pathway.

Interpretation bands for the pSOFA Score. Apply clinical judgement and local guidance.
