Gold Standards Framework Prognostic Indicator Guidance (GSF-PIG)

UK Gold Standards Framework triggers for identifying patients in last 6–12 months of life. Counts the number of generic + specific indicators present.

Score interpretation

Not yet on the supportive/palliative care register 0–1

→ Continue active management. Re-screen at routine review or with clinical change.

Consider GSF register — uncertain trajectory (~12 months) 2–4

→ Add to local supportive/palliative care register (green/amber phase). MDT discussion; advance care planning conversation; ReSPECT initiation.

Clearly approaching last year of life 5–7

→ GSF amber/red. Anticipatory care plan, preferred place of care, anticipatory medications, family communication. Hospice or community palliative referral.
