Score interpretation

Low symptom burden

→ Continue current management; reassess at each visit; address specific symptoms if any score >=4

Moderate symptom burden

→ Review all scoring >=4; adjust medications; specialist palliative input; consider syringe driver if multiple severe symptoms

High symptom burden

→ Urgent palliative care review; optimise all symptom management; consider inpatient hospice or hospital admission; family support; spiritual care
