→ Annual review at ACHD centre; echocardiogram, ECG, liver ultrasound, exercise test; anticoagulation (warfarin or NOAC); healthy lifestyle; prompt treatment of infections

Early Fontan failure signs — intervention may be needed

→ Cardiac catheterisation; Fontan conversion planning; diuretics for hepatic congestion; arrhythmia ablation; protein-losing enteropathy treatment (octreotide, budesonide); ACHD centre referral

Advanced Fontan failure

→ Heart transplant assessment; Fontan conversion to TCPC if appropriate; PDE5 inhibitor for PVR reduction; intensive monitoring; multidisciplinary ACHD team; transplant listing criteria assessment
