Subtle Anterior STEMI Calculator (4-Variable)
Distinguishes subtle anterior STEMI (OMI) from early repolarisation pattern using 4 ECG measurements from leads V2–V4 and aVR. Score > 22.9 = STEMI.
Score interpretation
Score ≤ 22.9: Pattern more consistent with early repolarisation. STEMI less likely.
→ Serial ECGs and troponins. If clinical suspicion high, cath lab activation still warranted.
Score > 22.9: Pattern consistent with subtle anterior STEMI / occlusion MI (OMI). Sensitivity ~91%, Specificity ~86%.
→ Immediate cath lab activation. Do not delay for troponin if clinical picture supports STEMI.
