DIPSS — Dynamic International Prognostic Scoring System for Myelofibrosis

Predicts overall survival in primary myelofibrosis. Can be applied at any time during the disease course.

Score interpretation

Low Risk 0–1

Low risk. Median OS not reached (~15+ years).

→ Watch and wait; symptom management. JAK inhibitor if symptomatic.

Intermediate-1 Risk 2–3

Intermediate-1 risk. Median OS ~14 years.

→ Symptom control. Consider JAK inhibitors for splenomegaly/symptoms.

Intermediate-2 Risk 4–5

Intermediate-2 risk. Median OS ~4 years.

→ JAK inhibitor therapy (ruxolitinib). Consider allo-SCT in eligible patients.

High Risk 6

High risk. Median OS ~1.5 years.

→ Urgent haematology review. JAK inhibitor; allo-SCT if eligible; clinical trial. Supportive care.
