WPSS — WHO Prognostic Scoring System for MDS

WHO-based prognostic scoring system for myelodysplastic syndromes. Incorporates WHO category, karyotype, and transfusion dependence.

Score interpretation

Very Low Risk 0

WPSS 0. Median OS ~136 months.

→ Watch and wait or EPO for anaemia. Annual review.

Low Risk 1

WPSS 1. Median OS ~63 months.

→ Supportive care, EPO, growth factors. Consider lenalidomide for del5q.

Intermediate Risk 2

WPSS 2. Median OS ~44 months.

→ Hypomethylating agents (azacitidine) may be indicated. Consider allo-SCT if eligible.

High Risk 3–4

WPSS 3–4. Median OS ~19 months.

→ Azacitidine or intensive chemotherapy. Allo-SCT for eligible patients. Clinical trial.

Very High Risk 5–6

WPSS 5–6. Median OS ~8 months.

→ Best supportive care or hypomethylating agent. Allo-SCT only if fit and suitable donor available. Palliative considerations.
