Glasgow Outcome Scale — Extended (GOSE)

Eight-point outcome measure for traumatic brain injury (Wilson 1998), used as primary endpoint in TBI trials and clinics. Assessed via structured interview.

Score interpretation

Death / vegetative state 1–2

→ End-of-life or chronic disorders of consciousness pathway (RCP 2020). Specialist neurorehabilitation MDT, advance care planning, family support.

Severe disability 3–4

→ Specialist inpatient neurorehabilitation; long-term care planning; carer support; spasticity, seizure and bladder management.

Moderate disability 5–6

→ Community neurorehabilitation; vocational rehabilitation; cognitive therapy; review of fitness to drive (DVLA).

Good recovery 7–8

→ Outpatient follow-up; address residual symptoms (post-concussive headache, sleep, mood). Reassure and re-integrate.
