IMPACT Score for Traumatic Brain Injury Outcome
International Mission for Prognosis and Clinical Trial design in TBI (IMPACT). Predicts 6-month outcome (death or vegetative state) in patients with moderate-severe TBI. Core, CT, and Lab models available.
Score interpretation
→ IMPACT Low (0–6): Predicted 6-month mortality/vegetative state <20%. Aggressive supportive care; ICP monitoring if indicated; early rehabilitation. Good functional outcome likely.
→ IMPACT Intermediate (7–12): Predicted mortality/vegetative state 20–50%. Intensive neurocritical care; goals of care discussion with family; consider ICP monitoring and decompressive craniectomy.
→ IMPACT High (>12): Predicted mortality/vegetative state >50%. Early goals-of-care discussion essential. Multi-disciplinary input; consider withdrawal of life-sustaining treatment if appropriate per family wishes.
