How it works
The constituent B vitamins act as essential coenzymes in carbohydrate, protein and fat metabolism and in energy production and nerve function.
Prescribing in practice
- Confirm the specific deficiency and choose an appropriate formulation, as high-dose pyridoxine taken chronically can cause peripheral neuropathy.
- Water-soluble vitamins are generally well tolerated and excess is largely excreted renally, but the individual constituents should be checked against the indication.
- In suspected Wernicke's encephalopathy, parenteral thiamine-containing high-potency preparations are required rather than a standard oral B complex.
Monitoring
Monitoring is primarily clinical, assessing resolution of deficiency symptoms, with biochemical testing reserved for specific deficiencies.
Counselling the patient
- Take as directed and do not exceed the recommended intake.
- Some products may colour the urine bright yellow, which is harmless.
