Acute Phase (Days 0–7)
Hyperacute: thrombolysis / thrombectomy per acute stroke pathway.
Admission to acute stroke unit.
First 24h: avoid mobilisation (AVERT 2015); after 24h, frequent gentle mobilisation as tolerated.
Dysphagia screen within 4h; NBM until safe swallow confirmed.
VTE prophylaxis: intermittent pneumatic compression (CLOTS-3); LMWH from 24h post-haemorrhagic stroke (per neurosurgery).
Specialist nurse + medical + nursing review; goal-setting within 1 week.
