Memorial Sloan Kettering Cancer Center prognostic model for metastatic renal cell carcinoma (prior to targeted therapy era, still used for first-line stratification).
Score interpretation
0 risk factors. Favourable risk. Median OS ~20 months (cytokine era).
→ VEGF-targeted therapy or IO-based combination. Consider watchful waiting in asymptomatic, low-volume disease.
1–2 risk factors. Intermediate risk. Median OS ~10 months.
→ IO/IO or IO/TKI first-line combination (per current ESMO/NICE guidelines). Clinical trial referral.
≥3 risk factors. Poor risk. Median OS ~4 months.
→ IO-based therapy. Goals of care discussion. Clinical trial strongly recommended. Early palliative input.
