Favourable Risk 0

0 risk factors. Favourable risk. Median OS ~20 months (cytokine era).

→ VEGF-targeted therapy or IO-based combination. Consider watchful waiting in asymptomatic, low-volume disease.

Intermediate Risk 1–2

1–2 risk factors. Intermediate risk. Median OS ~10 months.

→ IO/IO or IO/TKI first-line combination (per current ESMO/NICE guidelines). Clinical trial referral.

Poor Risk 3–5

≥3 risk factors. Poor risk. Median OS ~4 months.

→ IO-based therapy. Goals of care discussion. Clinical trial strongly recommended. Early palliative input.
