UPDRS Motor Score (MDS-UPDRS Part III)

Movement Disorder Society-Unified Parkinson's Disease Rating Scale (MDS-UPDRS) Part III motor examination. Assesses motor features of Parkinson disease. Scores 0 (normal) to 132 (severe). Used to titrate dopaminergic therapy.

Score interpretation

Mild Motor Impairment

→ UPDRS Motor Mild (0–5): Minimal motor impairment. Optimise dopaminergic therapy; exercise (aerobic + balance training); physiotherapy; occupational therapy for ADL strategies; speech and language therapy if dysarthric.

Moderate Motor Impairment

→ UPDRS Motor Moderate (6–14): Moderate motor impairment. Review and optimise levodopa/dopamine agonist dosing; consider DBS evaluation if motor fluctuations or dyskinesias; refer to MDT Parkinson clinic; falls risk assessment.

Severe Motor Impairment

→ UPDRS Motor Severe (≥15): Severe motor impairment. Advanced therapy review: apomorphine pump, levodopa-carbidopa intestinal gel, or DBS if appropriate candidate; palliative and rehabilitation input; carer support; care home assessment.
