Urine Protein:Creatinine Ratio (UPCR)

Spot urine protein:creatinine ratio estimates 24-hour urinary protein excretion without timed urine collection.

Score interpretation

A1 — normal to mildly increased (PCR < 15 mg/mmol)

→ Normal; no action required

A2 — moderately increased proteinuria (PCR 15–50 mg/mmol)

→ CKD monitoring; start RAAS inhibitor if diabetic; annual review

A3 — severely increased proteinuria (PCR > 50 mg/mmol)

→ RAAS inhibitor; nephrology referral; investigate cause (GN, diabetic nephropathy)

Nephrotic range proteinuria (>350 mg/mmol)

→ Urgent nephrology; renal biopsy consideration; oedema management; VTE prophylaxis

Interpretation bands for the UPCR. Apply clinical judgement and local guidance.
