Used in: Chronic Kidney Disease Acute Kidney Injury

Score interpretation

Low Risk CKD 2–3

G1-G2/A1. Low risk. eGFR normal or mildly reduced, no or minimal albuminuria.

→ Annual monitoring. Identify and treat reversible causes. Lifestyle modifications. Blood pressure <130/80 mmHg.

Moderate Risk CKD 4–5

Moderate risk. G3a/A1 or G1-G2/A2.

→ 6-monthly monitoring of eGFR and UACR. ACEi/ARB if proteinuria and diabetes/hypertension. Minimise nephrotoxins.

High Risk CKD 6–7

High risk. G3b or A2 with low eGFR.

→ 3-monthly monitoring. Nephrology review. Anaemia and bone disease assessment. Avoid NSAIDs and contrast agents.

Very High Risk CKD 8–9

Very high risk. G4-G5 or A3.

→ Nephrology specialist care. CKD education (dialysis options, transplant). Prepare for renal replacement therapy. Monthly monitoring.
