Topical Antifungal — Otomycosis

Clotrimazole Ear Drops
Brand names: Canesten Ear, Clotrimazole 1% Solution
Adult dose
Dose: 2–3 drops
Route: Otic
Frequency: Three times daily for 14–21 days
Max: 3 drops per ear three times daily
First-line treatment for otomycosis (fungal otitis externa). Most common causative organisms: Aspergillus niger (black spores), Aspergillus fumigatus, Candida albicans. Requires thorough aural toilet (microsuction) before application. Continue for minimum 2 weeks after symptom resolution.
Paediatric dose
Dose: 2–3 drops drops/kg

Same dose as adults; safe in children. Ensure aural toilet performed before use.

Clinical pearls
- Otomycosis diagnosis: black or white fluffy debris in the ear canal — Aspergillus niger produces characteristic black spore heads visible on otoscopy or microsuction
- Aural toilet (microsuction by ENT) is essential before and during treatment — antifungal drops cannot penetrate fungal debris
- Avoid topical antibacterial/steroid drops (gentamicin, hydrocortisone) — steroids promote fungal growth; if misdiagnosed as bacterial OE and treated with steroid drops, otomycosis worsens dramatically
- Prolonged antibiotic or steroid ear drop use is the most common precipitating factor for otomycosis
- Treatment failure or recurrence: consider systemic itraconazole or voriconazole in immunocompromised patients; re-examine for perforated TM
- Topical acetic acid (2%) is an alternative antifungal agent — changes pH of ear canal to inhibit fungal growth
Contraindications
- Perforated tympanic membrane (relative — limited data)
- Hypersensitivity to clotrimazole

- Local burning or stinging (transient)
- Local irritation
- Skin sensitisation (rare)

- Negligible at topical otic doses

- Symptom response at 2 weeks
- Repeat microsuction if debris accumulates
- TM integrity
