Diagnose
Obsessions (intrusive, unwanted thoughts / images / urges) + compulsions (repetitive behaviours / mental acts performed to reduce anxiety) — recognised as own thoughts (vs psychotic) + cause distress / impairment / time-consuming (>1h/day). Use Y-BOCS (severity 8–15 mild; 16–23 moderate; 24–31 severe; ≥32 extreme). Common subtypes: contamination + washing, checking, symmetry, intrusive (taboo) thoughts, hoarding (now distinct in DSM-5).
