Depression Assessment
PHQ-9 for severity: 1–4 minimal, 5–9 mild, 10–14 moderate, 15–19 moderately severe, 20+ severe. Risk assessment (suicidal ideation, plans, intent). Screen for bipolar disorder before prescribing.
