Logistic Organ Dysfunction Score (LODS)
7-organ-system dysfunction score for ICU patients. Based on worst values in the first 24 hours of ICU admission. Weighted for severity. Maximum score 22. Used for mortality prediction and benchmarking.
Score interpretation
LODS 0–4 — mild organ dysfunction; low predicted mortality
→ Standard ICU monitoring; organ support as clinically indicated; daily reassessment; target organ function recovery
LODS 5–9 — moderate multi-organ dysfunction; intermediate mortality
→ Active organ support (vasopressors, renal replacement, ventilation as needed); daily SOFA/LODS reassessment; ICU consultant review; family communication about prognosis; consider specialist input (renal, hepatology)
LODS ≥10 — severe multi-organ failure; high predicted mortality
→ Full ICU organ support; senior ICU consultant involvement; goals of care discussion with family; consider withdrawal of treatment discussions if irreversible; palliative care team involvement; ethical review if prolonged futile treatment
