Multiple Organ Dysfunction Score (MODS)
Quantifies the degree of dysfunction in 6 organ systems: respiratory, renal, hepatic, cardiovascular, haematological, and neurological. Predicts ICU mortality. Complements SOFA score.
Score interpretation
MODS 0-4. Minimal organ dysfunction. Predicted mortality < 5%.
→ Standard ICU care. Monitor for deterioration. Treat underlying cause.
MODS 5-8. Moderate dysfunction. Predicted mortality 10-25%.
→ Goal-directed therapy. Optimise organ support. Daily reassessment. MDT approach.
MODS 9-12. Severe dysfunction. Predicted mortality 25-50%.
→ Maximal organ support. ICU senior review. Prognosis discussion with family. Consider CRRT, vasopressors, mechanical ventilation as indicated.
MODS >= 13. Critical multi-organ failure. Predicted mortality > 50-75%.
→ Critical care escalation. Family meeting regarding prognosis. Treatment withdrawal assessment if clinically appropriate. Palliative care input if appropriate.
