Clinical criteria for the diagnosis of IBS based on symptom pattern. The original IBS diagnostic criteria, predating Rome. ≥3 of 6 criteria suggests IBS.

IBS Unlikely 0–2

Fewer than 3 Manning criteria — IBS diagnosis unlikely

→ Investigate other causes of symptoms; consider coeliac, IBD, colorectal cancer screening if alarm features

Consistent with IBS 3–6

≥3 Manning criteria — diagnosis consistent with IBS (75% sensitivity, 78% specificity)

→ Diagnose IBS if no alarm features (rectal bleeding, weight loss, age >50 with new symptoms, nocturnal symptoms); initiate IBS management
