Ulcerative Colitis Endoscopic Index of Severity (UCEIS)

Endoscopic scoring system for ulcerative colitis severity. Validated for assessing mucosal disease activity during colonoscopy.

Used in: Inflammatory Bowel Disease

Score interpretation

Remission 0–1

UCEIS 0–1 — mucosal remission

→ Maintain current therapy; consider step-down if in sustained remission

Mild-Moderate Activity 2–4

UCEIS 2–4 — mild to moderate endoscopic activity

→ Optimise 5-ASA or initiate/increase immunosuppressive therapy; repeat endoscopy at 3–6 months

Severe Activity 5–8

UCEIS 5–8 — severe endoscopic activity

→ Biologic therapy (infliximab/vedolizumab) or intensification; surgical consultation if failing medical therapy
