Ulcerative Colitis Endoscopic Index of Severity (UCEIS)

Endoscopic scoring system for ulcerative colitis severity. Validated for assessing mucosal disease activity during colonoscopy.

Score interpretation

Remission 0–1

UCEIS 0–1 — endoscopic remission

→ Maintain current therapy; consider step-down if in sustained remission

Mild Activity 2–4

UCEIS 2–4 — mild endoscopic activity (Ikeya 2016)

→ Optimise 5-ASA or initiate/increase immunosuppressive therapy; repeat endoscopy at 3–6 months

Moderate Activity 5–6

UCEIS 5–6 — moderate endoscopic activity (Ikeya 2016)

→ Escalate therapy (steroids, immunomodulator or advanced therapy) per specialist review

Severe Activity 7–8

UCEIS 7–8 — severe endoscopic activity (Ikeya 2016)

→ Biologic therapy (infliximab/vedolizumab) or intensification; surgical consultation if failing medical therapy
