Biophysical Profile (BPP) Score

Five-component fetal wellbeing assessment (Manning 1980): NST, fetal breathing, gross body movement, tone, and amniotic fluid volume. Each scored 0 or 2. Total 0–10. Used in third-trimester surveillance for high-risk pregnancies.

Score interpretation

Normal BPP 8–10

→ Reassuring. Reassess per local high-risk protocol (typically weekly or twice-weekly). No intervention.

Equivocal 6

→ Repeat in 4–24 h depending on gestation and risk. Deliver if ≥36 weeks; if <36 weeks repeat and consider corticosteroids if delivery becomes likely.

Abnormal — concerning fetal compromise 4

→ Deliver if ≥32 weeks. If <32 weeks, repeat same day, give corticosteroids and magnesium for neuroprotection, and deliver if persists.

Severely abnormal — suspected fetal acidaemia 0–2

→ Urgent delivery regardless of gestation if ≥26 weeks. Continuous CTG, antenatal corticosteroids, magnesium sulphate, and tertiary neonatal centre. Document maternal counselling.
