FIGO Staging for Ovarian Cancer (2014)

2014 FIGO (International Federation of Gynaecology and Obstetrics) staging system for ovarian, fallopian tube, and peritoneal cancer. Used for prognosis and treatment planning.

Score interpretation

Stage I — Localised 1–2

FIGO Stage I — tumour confined to ovary/ovaries

→ Stage IA/IB: fertility-sparing surgery appropriate if reproductive age; total abdominal hysterectomy + bilateral salpingo-oophorectomy + staging (omentectomy, lymph nodes, peritoneal biopsies); adjuvant chemotherapy for IC; platinum-based chemotherapy (carboplatin + paclitaxel) if high-grade

Stage II — Pelvic Extension 3

FIGO Stage II — tumour involves one or both ovaries with pelvic extension

→ Optimal cytoreductive surgery (aim for no residual disease); platinum-based chemotherapy 6 cycles; consider HIPEC in selected centres; MDT discussion; genetic testing (BRCA1/2, Lynch)

Stage III — Abdominal Spread 4–5

FIGO Stage III — tumour involves one or both ovaries with peritoneal spread beyond pelvis

→ Primary debulking surgery (PDS) if resectable; neoadjuvant chemotherapy (NACT) then interval debulking if upfront surgery not feasible; carboplatin + paclitaxel × 6 cycles ± bevacizumab; PARP inhibitor maintenance (olaparib/niraparib) if BRCA mutated; MDT; clinical trial consideration

Stage IV — Distant Metastases 6

FIGO Stage IV — distant metastases beyond peritoneal cavity

→ NACT then interval debulking; carboplatin + paclitaxel ± bevacizumab; maintenance therapy; palliative care involvement; BRCA/HRD testing for PARP inhibitor eligibility; clinical trial; comprehensive MDT
