FRAX Score (Fracture Risk Assessment)

Estimates 10-year probability of hip and major osteoporotic fracture. Guides DEXA scan referral and bisphosphonate initiation per NOGG/NICE guidelines.

Score interpretation

Low fracture risk

→ Lifestyle advice: calcium 700-1200mg/day dietary, vitamin D 800-1000 IU/day; weight-bearing exercise; fall prevention; reassess in 5 years

Moderate risk — DEXA scan recommended

→ DEXA scan; if T-score <=-2.5 (osteoporosis) or <=-1.0 with multiple risk factors: bisphosphonate (alendronate 70mg weekly first-line); calcium and vitamin D supplementation; NOGG threshold calculator

High fracture risk — treat

→ Bisphosphonate without awaiting DEXA if >75 and low-energy fracture; alendronate 70mg weekly; risedronate or zoledronate (annual IV) if GI intolerance; denosumab 60mg SC 6-monthly for severe osteoporosis; post-fracture liaison service referral
