PELOD-2 — Paediatric Logistic Organ Dysfunction Score

Simplified second-version paediatric organ dysfunction score (Leteurtre 2013). Range 0–33. Daily mortality predictor on PICU. Drives benchmarking and trial endpoint reporting.

Score interpretation

Low organ dysfunction (mortality ~1%) 0

→ Continue current PICU management. Consider step-down once weaning successful.

Moderate (~5–10%) 1

→ Aggressive support of failing organs; reassess daily. Escalate as needed (CRRT, ECMO consideration if respiratory failure).

High (~25%) 2

→ Senior PICU and parental discussion regarding intensity of support; consider transfer to ECMO centre. Multidisciplinary input (cardiology, nephrology).

Very high (>50%) 3

→ Family meeting on prognosis and goals of care; palliative-care team involvement; consider treatment limitation discussions per RCPCH framework.
