Shock Index, Pediatric Age-Adjusted (SIPA)

Age-adjusted Shock Index for paediatric patients. HR/SBP ratio with age-specific thresholds. Predicts need for blood transfusion, massive transfusion, and mortality in paediatric trauma.

Score interpretation

Normal SIPA (Age 13–16) 0–0.79

SIPA below age-specific threshold — normal haemodynamic state for age

→ Continue standard trauma assessment; no immediate need for massive transfusion protocol activation

Elevated SIPA — Assess Age ≥ 0.8

SIPA at or above threshold for age — haemodynamic compromise

→ Activate massive transfusion protocol; urgent surgical review; resuscitation with blood products; FAST exam; IV access × 2
