Urine Protein:Creatinine Ratio (UPCR)
Estimates 24-hour urinary protein excretion from spot urine protein and creatinine. Used to quantify proteinuria in CKD, nephrotic syndrome, and pre-eclampsia.
Score interpretation
Normal proteinuria (<15 mg/mmol)
→ Normal; no action required
Microalbuminuria / mild proteinuria
→ CKD monitoring; start RAAS inhibitor if diabetic; annual review
Significant proteinuria (A2-A3)
→ RAAS inhibitor; nephrology referral; investigate cause (GN, diabetic nephropathy)
Nephrotic range proteinuria (>350 mg/mmol)
→ Urgent nephrology; renal biopsy consideration; oedema management; VTE prophylaxis
