Clinical Pulmonary Infection Score (CPIS) for VAP
Diagnoses ventilator-associated pneumonia (VAP) using clinical, radiological, and microbiological criteria. Score >6 suggests VAP.
Used in: Pneumonia
Score interpretation
VAP Unlikely 0–5
CPIS ≤6 — VAP unlikely; consider stopping antibiotics if initiated empirically
→ Monitor clinically; de-escalate antibiotics if started; consider alternative diagnoses
VAP Likely 6–12
CPIS >6 — VAP likely
→ Continue/initiate broad-spectrum antibiotics (covering gram-negatives and MRSA if risk factors); bronchoalveolar lavage for culture; minimise ventilator days
Interpretation bands for the CPIS. Apply clinical judgement and local guidance.
