Respiratory Emergency Medicine Cardiology Strong — ADJUST-PE trial, 3346 patients; endorsed by ESC PE guidelines

Age-Adjusted D-Dimer Threshold for PE

Calculates the age-adjusted D-dimer threshold for excluding PE in patients over 50. Reduces unnecessary CTPA in elderly patients with low-to-moderate pre-test probability. Formula: age × 10 μg/L.

Score interpretation

Below Age-Adjusted Threshold — PE Excluded 0

D-dimer is below the age-adjusted threshold (age × 10 μg/L for patients > 50 years; 500 μg/L for ≤ 50 years). PE safely excluded in low-to-intermediate pre-test probability.

→ CTPA not required. No anticoagulation needed. Pursue alternative diagnoses.

Above Age-Adjusted Threshold — CTPA Indicated 1

D-dimer exceeds the age-adjusted threshold. PE not excluded.

→ Proceed to CTPA. Consider empirical anticoagulation if high pre-test probability and imaging delayed > 4h.
