Rapid Shallow Breathing Index (RSBI)
Predicts successful extubation in mechanically ventilated patients. RSBI = respiratory rate / tidal volume (L). Values <105 predict weaning success.
Score interpretation
RSBI <80 — strong predictor of weaning success
→ Proceed with extubation if other criteria met; spontaneous breathing trial successful
RSBI 80–105 — borderline; weaning success possible
→ Reassess clinical picture; ensure adequate cough, secretion clearance, and neurological status before extubation
RSBI >105 — predictor of weaning failure (shallow rapid breathing pattern)
→ Continue ventilator support; address reversible causes (pain, anxiety, bronchospasm, secretions); repeat SBT in 24h
