Mepivacaine hydrochloride is an intermediate-acting amide local anaesthetic used for infiltration and regional nerve block anaesthesia, including in dentistry.

How it works
It reversibly blocks voltage-gated sodium channels in nerve fibres, inhibiting the initiation and conduction of nerve impulses.
Prescribing in practice
- As with all amide local anaesthetics, inadvertent intravascular injection risks systemic toxicity affecting the central nervous system and heart; aspirate, inject incrementally, and keep resuscitation facilities available.
- It produces less vasodilatation than some local anaesthetics, so adequate anaesthesia may be achieved without added vasoconstrictor.
- It must not be used for intravenous regional anaesthesia (Bier's block).
Monitoring
Monitor cardiovascular and neurological status during and after injection for features of local anaesthetic systemic toxicity.
Counselling the patient
- The numbed area will lack sensation for a period; avoid biting the lip, cheek, or tongue after dental use.
- Report dizziness, perioral numbness, or ringing in the ears without delay.
