How it works

Benzoic acid provides fungistatic activity while salicylic acid acts as a keratolytic, promoting desquamation of infected keratin so the fungistatic action can take effect.

Prescribing in practice

  • The salicylic acid content can cause local irritation and should be used cautiously on broken or inflamed skin and avoided on large areas.
  • Modern azole and allylamine antifungals are generally more effective and better tolerated, so this preparation is now largely a second-line option.
  • Avoid contact with the eyes and mucous membranes.

Monitoring

Review the affected area clinically to confirm resolution of infection and to detect local irritation.

Counselling the patient

  • Apply to the affected area and continue for the recommended period after the skin looks better.
  • Keep the area clean and dry, particularly between the toes.
  • Stop and seek advice if marked irritation occurs.

Evidence & guidelines

Whitfield's ointment is an established traditional topical antifungal, though newer agents are generally preferred where available.
